If you have any queries about entries that you see in your medical record when viewing them online (eg using the NHS App), then please note the following:

Our priority is of course providing clinical care to our patients. Reviewing queries relating to medical records may take time and is highly unlikely to be urgent. As a result we have put in place a system that will enable you to query entries in your medical record that doesn't distract our clinical team from their work. Please do not phone to speak to a GP or book an appointment to discuss your medical record.  Instead, we ask that you complete an e-Consultation using the "administrative" option.  Your request will be received and actioned at the earliest opportunity and you will receive a response from us via the eConsultation system. Depending on our workload and the nature of your query it may take several days, or longer, to complete work on your request.
